Job Description – Part-Time Clinical Home Nurse (LVN) for Group Homes (Texas)

Position Title: Licensed Vocational Nurse (LVN)

Reports To: Administrator / Executive Director

Location: DFW Veranda Assisted Living – Group Homes (Texas)

Schedule: Part-Time, flexible schedule with on-call responsibilities as needed

Position Summary

The Part-Time Clinical Home Nurse (LVN) provides professional nursing care and oversight across multiple small group homes (each serving fewer than 8 residents) under DFW Veranda Assisted Living. This role focuses on conducting house calls, overseeing medication administration, performing health assessments, and ensuring residents receive safe, compassionate, and person-centered care. The LVN supports caregivers in maintaining compliance with Texas Health and Human Services Commission (HHSC) regulations while promoting quality of life and well-being for all residents. Homes located in Carrollton, Arlington and Fort Worth.

Key Responsibilities

· Resident Assessments & Care Planning

· • Perform nursing assessments for new admissions, annual reviews, and as resident conditions change.

· • Develop and update individualized service plans that reflect residents’ physical, emotional, and social needs.

· • Communicate assessment findings to physicians, families, and administrators for care coordination.

· Medication & Health Oversight

· • Administer prescribed medications and treatments per physician orders and HHSC standards.

· • Oversee and delegate medication assistance to trained caregivers or medication aides.

· • Monitor residents for medication side effects or health changes, and document findings accurately.

· • Ensure medications are stored, labeled, and disposed of properly per Texas regulations.

· House Calls & Clinical Support

· • Conduct scheduled and as-needed home visits to evaluate residents’ health and respond to medical needs.

· • Provide basic nursing care, wound care, and vital sign checks as required.

· • Collaborate with physicians and healthcare providers for ongoing medical management.

· • Support emergency response and communicate urgent medical concerns promptly.

· Staff Guidance & Training

· • Provide caregivers with clinical supervision, feedback, and mentoring.

· • Conduct required HHSC staff training on infection control, emergency preparedness, and resident rights.

· • Observe and ensure caregivers follow safe care practices and facility procedures.

· Regulatory Compliance & Documentation

· • Ensure compliance with Texas Administrative Code, Title 26, Chapter 553 (Assisted Living Licensing Standards).

· • Maintain accurate documentation of assessments, progress notes, and incident reports in the Electronic Health Record (EHR).

· • Support HHSC inspections, audits, and surveys.

· • Promote resident safety, infection prevention, and emergency preparedness standards.

· Communication & Coordination

· • Act as the nursing liaison between residents, caregivers, families, and physicians.

· • Document communication and care updates clearly and promptly.

· • Utilize the organization’s online timekeeping system to record shifts accurately.

· • Coordinate care schedules with the Administrator to ensure coverage and continuity.

Qualifications

· Required

· • Current Texas LVN license in good standing.

· • Minimum 2 years of experience in home health, assisted living, or residential care nursing.

· • Knowledge of HHSC regulations for small assisted living and group home operations.

· • Strong assessment, communication, and documentation skills.

· • Must pass background check, TB test, and drug screening.

· • Proficiency in EHR systems and online timekeeping software.

· • Reliable transportation for travel between group homes.

· Preferred

· • Specialized training in dementia care, palliative care, or gerontology.

· • Prior experience in a small residential or group home setting.

· • CPR and First Aid Instructor certification.
